Please take note of this important information in the event of a claim:

- Claims must be reported directly to your insurance company.
- Claims may NOT be reported online.

Please be prepared to provide the preliminary information listed below when you call the insurance company to report a claim.
- Policy number.
- Name, address and phone number for all parties involved in the accident.
- Facts surrounding the claim, including date, time and location.
- Policy report number and name of the police agency.
- Information for the vehicle that was involved in the accident.
- Year, Make, Model, Vehicle Type and Serial #.
- Information for the driver that was involved in the accident.
- Driver's name and driver's license number.
- Extent of injuries of all parties involved.
